This is being rolled out in Hillingdon with the Metocare and Celandine PCN being the first to piolet the service. This free service is to patients monitor and manage their condition remotely.

WEBCare Home Support Team Eligibility: The service is open to all residents within a Hillingdon Care Home, Extra Care Housing, Mental Health and Learning Disability Homes. Patients requiring acute visiting appointments will be picked up via the matron’s morning triage call and booked in directly. Practices can book appointments for their patients.
