WEBThe SRA Health and Dental Plans are annual plans and therefore coverage can only be terminated on the annual renewal date of July 1st. If you decide to terminate your coverage, please advise GMS in writing no later than June 15, 2022.

WEBThis Schedule of Benefits forms part of this booklet and must be read in conjunction with the rest of this booklet. Dental Limitations: Members must purchase a health plan in order to be eligible to purchase a dental plan. Dental Care Benefit Maximum: $1,200 combined per person per Policy Year.
