Report your child's vaccinations
WEBTo report a vaccination, you'll need to provide the: Child's Ontario Health Card (OHIP) number or Ontario Immunization ID. Child's full name, address, and date of birth. Parent or guardian's name. Date the child was vaccinated. Vaccine the child received. Name of the child's school (if registered). View or report vaccinations.

COVID-19 vaccines
WEBAbout the COVID-19 vaccine and where to get the vaccine. Health Canada evaluates, licenses, and approves the COVID-19 vaccines for use in Canada. COVID-19 vaccines are available to anyone 6 months or older in Ontario for free, regardless of citizenship or immigration status. You do not need an Ontario health card to get the vaccine.
School vaccination program
